Collaborative primary care pharmacy services are provided within a progressive medical group that is transforming health care delivery through the patient centered medical home model. Residents will have direct patient care responsibilities across a spectrum of primary care practice environments including clinic, telephonic and e-health and will gain experience in delivery of team-based care. Core disease states include: HTN, DM, CAD, hyperlipidemia, HF, asthma/COPD, migraine, pain, and depression/anxiety. This is an ASHP-Accredited PGY-2 residency; prior PGY1 pharmacy residency is required.
