Position Description
The Transplant Clinical Pharmacy Specialist will provide clinical pharmacy services to the Renal Transplant service at the State University of New York (SUNY) Downstate Medical Center in Brooklyn, New York. The specialist will work as part of the multidisciplinary transplant team in order to deliver safe, appropriate, comprehensive and cost-effective drug therapy for pre-transplant and post-transplant patients. The individual will serve on committee/task forces and participate in projects related to transplant medication use such as drug utilization evaluations and performance improvement initiatives. Responsibilities also include providing educational programs for physicians, nurses, nephrologists, physician assistants, pharmacists, patients, and pharmacy residents and students.

Duties and Responsibilities:
• Participate in activities during pre-, peri-, and post-transplant phases of patient care
• Educate potential transplant recipients and living donors on medications likely required for post-transplantation or post-donation in pre-transplant clinic
• Assess potential transplant recipients for medication difficulties and present findings to Transplant Selection Committee
• Closely follow and make recommendations to all new transplant recipients and living donor when rounding with the Renal Transplant team
• Provide medication discharge counseling along with a customized medication planner
• Evaluate newly-transplanted recipients within one month of discharge for difficulties experienced with medication adherence in the transplant clinic
• Follow all post-transplant patients admitted to the inpatient renal transplant service
• Participate in projects related to transplant pharmacotherapy use such as drug utilization reviews
• Precept and mentor Pharmacy residents and students during the Transplant rotation
• Serves as voting member of Pharmacy and Therapeutics Committee

Required Qualifications:
• NYS Pharmacist Licensure or Eligible for NYS Pharmacist Licensure
• Doctorate of Pharmacy Degree from an accredited College of Pharmacy
• Completion of PGY1 Pharmacy Residency
• Completion of PGY2 Pharmacy Residency in Solid Organ Transplantation by the time of employment
