• To effectively fulfill this position, candidate must meet the following requirements:
o Consistently demonstrates appropriate and professional communication behavior towards patients and other health care professionals.
o Possesses strong critical/analytical thinking, organizational, communication and motivational skills.
o Efficient utilization of Word, PowerPoint and Excel required.
o Provides comprehensive pharmaceutical Transitions of Care to pertinent patient as needed:
Performing medication reconciliations on selected patients at admission and discharge as needed.
Rounding with hospitalists as needed and making medication recommendations based on best practice.
Collaborating with Case Managers and Care Managers to identify potential barriers to discharge.
Referring qualifying patients to MHS’s pharmacy-driven outpatient Comprehensive Health Management Service.
Providing patients and/or caregivers with drug therapy and disease-state counseling.
Collaborating with primary care providers, home health care agencies, rehab and skilled nursing facilities.
o Collects and analyzes data on transitions of care service line.
o Leads and participates in teaching programs within the Pharmacy Department and other education as related to Transitions of Care.
o Precepts and mentors PGY1 residents as well as pharmacy students.
o Actively participates in TOC team meetings, hospital committees, and conferences.
o Maintains annual mandatory education requirements specific to position as mandated by Martin Health System.
o Performs all duties of Decentralized/Centralized Clinical Pharmacist as needed.
o Must possess a degree from a College of Pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
o Current/Active State of FL Pharmacist License required
o Doctor of Pharmacy degree and/or completion a pharmacy residency with experience in Transitions of Care or equivalent Transitions of Care practice experience preferred.
o Board Certified Pharmacotherapy Specialist certification or equivalent required. If not already certified, must obtain within 2 years of date of hire.
