Clinical Pharmacist – Liver Transplant/Hepatology
TN-Nashville
8-hour Day Shifts with rotation every 4th weekend and holidays

This Clinical Pharmacist will be a member of the Transplant Clinical Pharmacy Team. This team provides care in a variety of areas, including renal/pancreas, liver, lung, heart, and pediatrics. This pharmacist will be one of two Liver Transplant/Hepatology Clinical Pharmacists. Pharmacists are an integral aspect of patient care at Vanderbilt, with a strong presence and excellent relationships with physicians and nursing partners across all specialties. Collaboration and integration with pharmacy, nursing and medical staff, and excellent communication skills are essential.

This pharmacist will have the following clinical and administrative responsibilities:
• Round consistently with the medical team
• Participate in clinic and provide clinical pharmacy services and oversight for safe use of medication. This includes prescribing, dispensing, administering, and monitoring of medications.
• Interpret pharmacokinetic/therapeutic drug monitoring data
• Screen for drug interactions, therapeutic duplications, omissions, and monitor patients for therapeutic and/or toxic response to medications
• Provide coordination and oversight of transition of high-acuity patients from inpatient to ambulatory
• Develop and participate in pharmacy-related research projects and quality improvement initiatives, etc. within the program
• Provide oversight of transplant staff education for critical elements of medication use in patient care and clinical research
• Educate medical, nursing, and pharmacy staff about the management, prescribing and monitoring of transplant therapies
• Participate in the education of pharmacy students and PGY1 and PGY2 pharmacy residents
• Present scholarly materials at educational conferences for the transplant program and pharmacy professional meetings and organizations (State/Local, National)

Position requirements:
• Bachelor's Degree and 5 years relevant experience plus Bachelor's Degree or PharmD plus five (5) years relevant experience or PharmD plus completion of ASHP - Accredited PGY1 residency training program.
• Tennessee Pharmacist licensure
• PGY2 in Solid Organ Transplant or equivalent practice experience

Preferred qualifications:
• Doctor of Pharmacy or Master of Science
• 3 years of transplant experience post-PGY2 residency preferred
• Board Certification in Pharmacotherapy
• Experience working in inpatient and outpatient settings in an academic medical center
• Experience with Epic®
