Position Description
Full-time, Monday thru Friday position for a Clinical Pharmacist, Ambulatory Care within the Mercy Health Physician Partners primary care, PCMH offices (Mercy Health Saint Mary’s health system in Grand Rapids, Michigan; part of Trinity-Health).

Responsibilities include:
• Disease management within institution approved collaborative practice agreements (diabetes, hypertension, hyperlipidemia),
• Referral-based Comprehensive Medication Review with recommendations to physicians,
• Daily ad hoc informal consultations to improve pharmacotherapy effectiveness, safety, and cost as well as participating in team based care within a multidisciplinary model.

Additional activities include coverage in our Anticoagulation Management Service and other multi-disciplinary initiatives focused on particular high-risk patients or populations (e.g., depression, frail elderly). Support of related committees (e.g., Quality Committee) and identification and development of additional clinical services or practice improvements are also expected.

• PharmD from an ACPE-accredited school/college of Pharmacy
• PGY-1 Residency
• PGY-2 Ambulatory Care Residency (strongly preferred) or commensurate practice experience in ambulatory care with direct pharmacotherapy management responsibilities;
• BPS Specialty Certification (Pharmacotherapy or Ambulatory Care, preferred, or certification within 1 year of hire)
• Licensed (pharmacist and controlled substance) in the State of Michigan, or eligible
