HealthEast St. Joseph’s Hospital in St. Paul, Minnesota has recently extended capacity of inpatient mental health bed count to 105. This expansion includes geriatric psychiatric and inpatient chemical dependency care. We are uniquely positioned with strong provider support to provide medication therapy management services in our outpatient mental health hospital-based clinic and are currently seeking qualified candidates for this position.

The clinical pharmacy specialist role will be comprised of both inpatient and outpatient responsibilities. As the volume in the outpatient area grows, we anticipate the position will likely evolve into separate roles. The ideal candidate will be innovative, flexible to change, self-directed, and self-motivated.

This clinical pharmacy specialist position will be embedded in the outpatient mental health clinic. In this setting, the role of the pharmacist is to provide general medication reconciliation, assess the patient’s clinical status and medications, optimize medication therapies, ensure appropriate lab monitoring, and provide patient/caregiver education. Through the provision of a collaborative practice agreement, the pharmacist will provide pharmacy services for psychiatrists and nurses seeing patients both in coordination with the providers or individually for medication therapy management. The pharmacist will provide education to physicians, mid-levels, and nurses.

To support the inpatient mental health and chemical dependency units, the clinical pharmacy specialist will be responsible for the provision and oversight of pharmacotherapy practice, policies, procedures and education. The specialist will serve as a resource for the pharmacy team, nursing, and prescribers for questions/issues related to pharmacotherapy; analyze literature to support usage and contact prescribers as needed.

In addition to the inpatient/outpatient clinical functions, the position will support regulatory efforts related to mental health throughout the health system. Working collaboratively with providers, nurses, and other health care providers to initiate and assist in protocol development and implementation in specialty practice area to optimize medication utilization, improve patient outcomes and improve cost effectiveness.
