Position Description
Provides quality pharmaceutical care defined as being responsible for assuring the provision of safe, efficacious and cost-effective drug therapy for all patients. The clinical pharmacist works in a multidisciplinary practice and collaborates with all team members utilizing a collaborative practice based care team model. The pharmacist provides face-to-face and one-on-one phone based disease management with patients. The pharmacist will utilize integrated and routine communication with all health care professionals and the patients concerning medical care and quality of life with the emphasis placed on the monitoring and documentation of specific pharmacotherapy outcomes. Provides assistance and in-depth drug information to medical personnel; is responsible for Medication Therapy Disease Management(MTDM) activities related to oral chemotherapy. The clinical pharmacist will consult with the patient to identify, resolve and prevent medication related problems. The clinical pharmacist assesses each of the patient’s medications for indication, effectiveness, safety and adherence issues and orders lab tests in the assessment process. The clinical pharmacist will follow up with the patient to titrate/manage/monitor the patient’s medication therapy to ensure positive outcomes


Bachelor of Science in pharmacy or doctor of pharmacy (Pharm D) degree from an accredited college and current licensure or eligibility for licensure by the Pennsylvania State Board of Pharmacy required.

In the absence of a PharmD., a Bachelor of Science in pharmacy with appropriate clinical pharmacy experience will substitute.
