Faculty Position Transition of Care
Based on Experience
Tenure Track Clinical Faculty
Wilkes-Barre, PA
Position Description
Wilkes University’s Nesbitt School of Pharmacy, Department of Pharmacy Practice in collaboration with Geisinger invites applicants for a full time, calendar year faculty position at the rank of Assistant or Associate Professor of Pharmacy Practice. This position is fully funded by Wilkes University and is tenure-track. The practice area is focused on transitions of care with a practice setting within Geisinger’s inpatient Wyoming Valley Medical Center and an outreach component to surrounding ambulatory clinics. The clinical practice faculty will provide comprehensive medication management for patients with high risk of hospital readmission. This can include face-to-face patient interaction before hospital discharge and/or during ambulatory clinic visits, as well as telehealth and home visits.

Geisinger is an integrated health system serving patients across the state of Pennsylvania and parts of New Jersey. Geisinger is known for its innovative culture of patient centric multidisciplinary team-based care and uses collaborative practice agreements to facilitate the pharmacist’s role on the health care team. Geisinger’s Center for Pharmacy Innovation and Outcomes has the mission “to conduct, implement, and disseminate research to optimize medication use,” making this collaboration a valuable resource to achieve faculty clinical practice and scholarship goals.

The School of Pharmacy has a dynamic, comprehensive and innovative approach to pharmacy education. Our mission is "to develop pharmacists who will provide high quality health care and to make meaningful contributions to the science and practice of pharmacy.” In addition to experiential activities, the successful candidate will participate in didactic teaching, provide service to the school, serve as a student advisor, and engage in meaningful scholarship. Didactic teaching can include both small and large classes as well as professional labs. Campus responsibilities such as serving on school committees can be facilitated with remote access to preserve time for clinical practice activities.

Successful candidates should have a Doctor of Pharmacy degree and have ideally completed a PGY-2 pharmacy residency or possess equivalent clinical experience in the practice area of ambulatory care, internal medicine, or transitions of care. Additionally, experience with a health system-based process, project management, quality improvement initiatives, and exposure to patients at increased risk of readmissions is desired. At Wilkes, excellence in teaching is the primary consideration for tenure and promotion. Salary is commensurate with qualifications and experience.

When applying, please upload a letter of intent, a current curriculum vitae and the names and contact information for at least three professional references.
