Position: Ambulatory Hematology/Neuro-oncology Clinic

Colorado pharmacist licensure
PGY1 Pharmacy practice residency required
PGY2 in Pediatrics or Oncology strongly preferred
Board Certification in Pediatrics or Oncology strongly preferred

The ambulatory oncology clinic pharmacist will provide clinical pharmacy services to oncology and non-oncology patients in the ambulatory care setting. The Center for Cancer and Blood Disorders, the CCBD, provides infusion services to 14 specialty clinics. The clinics include: oncology, bone marrow transplant, neuro-oncology, hematology, nephrology, gastroenterology, neurology, endocrinology, dermatology, metabolic, pain, pulmonary, renal, rheumatology and special care.

The ambulatory oncology clinic pharmacist responsibilities:
• Ensure safe, appropriate, cost effective oncology therapies according to the established policies, procedures and protocols
• Facilitate chemotherapy orders and the preparation and administration of chemotherapy products (including chemotherapy medication order verification)
• Monitor and evaluate patient specific pharmacotherapeutic regimens
• Participate in quality improvement/process improvement initiatives that improve the quality of pharmacy services to the infusion center
• Oversee a seamless transition of pharmacy services between inpatient/outpatient setting
• Medication reconciliation/Discharge medication teaching
• Facilitate drug information requests from healthcare providers and patients
• Serve as a member of hospital committees involved evaluating oncology related topics such as treatment algorithms, algorithms for complications due to chemotherapy
• Establish collaborative relationships with the infusion center staff and other health professionals associated with the infusion center
• Promote education and training of pharmacy students, pharmacy residents and other healthcare professionals
