Sr Pharmacist- Inpatient Transplant
Salary Range: $6096.80 to $6866.40 Bi-weekly

Responsibilities
The Senior Transplant Pharmacist works to facilitate comprehensive medication management and oversight for solid organ transplant patients and inpatient pulmonary hypertension patients in both the hospital and clinics. Activities include participation in daily, rounds, review and verification of inpatient orders, facilitation of complex inpatient medication orders, coordination of discharge medication procurement and education, and medication reconciliation for all patients on the Transplant and Adult pulmonary services. In the outpatient areas, the Senior Transplant Pharmacist participates in pre-transplant assessment and follow-up care for patients receiving their pre- and post-transplant care at UC Davis Health. Additionally, the Senior Transplant Pharmacist periodically assists with patient assessment and medication procurement in the Transplant RAMP (Refill Adherence, Mailing Program) Pharmacy.

Qualifications
REQUIRED
• Current California Pharmacist License
• Transplant clinical pharmacy experience
• Recent clinical pharmacy experience (inpatient and/or ambulatory clinic) (minimum of 5 years in lieu of PGY2 residency training)
• Expert communication skills to make independent decisions and resolve minor problems as they arise
• Demonstrated working knowledge of, and skill to apply, practices, principles and techniques of all aspects of patient care related to solid organ transplantation
• Pharmacist Board Certification required or willingness to obtain certification in specialty area (if currently available)
• Must be able to accurately perform pharmacy calculations
• Experience obtaining information from a patient or patient's chart to create a management plan
• Experience communicating detailed patient care plans to healthcare professionals
• Solid understanding of relevant clinical practice and regulatory guidelines related to the care of solid organ transplant patients
• High level written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English
• Experience and ability to work with and direct health care professionals
• Solid decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached
• Demonstrated problem-solving skills to independently recognize and define a problem; identify the resources available to help solve the problem; create and implement viable solutions; and follow through to ensure the problem is resolved to the satisfaction of all parties

PREFERRED
• PGY1 pharmacy practice residency preferred
• PGY2 Solid Organ Transplant residency preferred
• Recent clinical pharmacy experience (inpatient and/or ambulatory clinic) (minimum of 5 years in lieu of PGY2 residency training) preferred
• Ability to supervise technicians and establish priorities of work-flow preferred
