The successful candidate will provide medication management services in Medical Oncology clinic areas. Designs, implements, monitors, evaluates and modifies patient pharmacotherapy to ensure that medication is indicated, effective, safe and convenient. This individual will complete documentation and billing processes for pharmacist activities and participates in collaborative practice and/or protocol-driven practice, as appropriate.

Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Equivalent experience includes at least two years of relevant pharmacy practice experience.
