Position Description
Advocate Medical Group has an open Ambulatory Care Clinical Pharmacist position in Chronic Disease Management in the South Chicagoland area. It is a unique, progressive clinical position that fosters trained Clinical Pharmacists to work at the top of their license in a collaborative and multi-disciplinary team.

PharmD degree
PGY1 Residency (required)
PGY2 Residency with Ambulatory Care focus (preferred)
Board Certification in Ambulatory Care or Pharmacotherapy (preferred)

• Knowledge of group practice management
• Strong clinical understanding of general pharmacotherapy and evidence-based practice/guidelines
• Strong writing and communications skills
• Strong statistical/analytical skills
• Must possess qualities of organization, prioritization, and problem solving skills and demonstrate self-directed, results-oriented skills
• Active Illinois pharmacist license
• Local travel to various sites of care (minimal)

The successful candidate will build a Chronic Disease Management practice that will grow to include 500 to 1000 patients. The practitioner will be expected to care for 8-10 patients per day, using the standards of comprehensive medication management services which include an assessment of all of a patient's medications, the development of individualized care plans, and follow-up evaluation to determine actual outcomes. Documentation of all patient care is expected. The successful candidate will work collaboratively with primary care physicians, specialists, advanced practice clinicians, and other clinical pharmacists to deliver optimal drug therapy to patients.

• Direct patient care including Heart Failure, Diabetes, Hypertension, Dyslipidemia, COPD/Asthma and Anticoagulation.
• Planning and direction of the clinical activities to ensure the provision of quality patient care and to meet present and future demands.
• Ability to educate the medical staff and other team members, patients, parents/guardians, and students regarding ambulatory care pharmacology and therapeutics, along with updates in literature and guidelines.
• Ability to assume other related responsibilities as assigned.
