Ambulatory Care PRN Focus Session — Heart Failure Management: New Guidelines and Practice-changing Literature

The Ambulatory Care PRN Education Committee presents a session addressing clinical practice guidelines, primary literature, and practice pearls regarding heart failure management. More specifically, the presentation will include a disease state management update focusing on changes in the most recent guidelines (particularly changes in the management of HFpEF and new categories of HFmrEF and HFimpEF), as well as practice changing studies that have been recently published. Then the focus will shift to a deeper dive “beyond the basics”, applying this recent literature to clinical “grey areas” and special patient populations. The final portion of the session focuses on growing opportunities for pharmacists in diuretic management and remote patient monitoring. The purpose of this session is to provide an overview of the existing clinical evidence, resources, and practice opportunities for the ambulatory care pharmacist.

Learning Objectives

1. Summarize key changes in heart failure classification and management recommended in the most recent heart failure guideline update and heart failure with preserved ejection fraction expert consensus pathway.

2. Outline key takeaways from recent practice-changing heart failure studies and how they may impact future heart failure management recommendations.

3. Select the appropriate guideline recommendations to optimize care for special populations (e.g., those with concomitant chronic kidney disease, obesity or iron deficiency, older adults, or those with continued symptoms in spite of standard goal-directed medical therapy).

4. Describe the role of remote patient monitoring, injectable diuretic formulations, and opportunities for pharmacist-managed outpatient diuretic therapy.
