Ambulatory care clinical pharmacists commonly encounter and manage patients with geriatric disease states and syndromes, as well as patients with various types of renal dysfunction. ACSAP 2018 Book 3 (Nephrologic and Geriatric Care) presents evidence-based updates on the management drug therapy in the setting of the older adult and ambulatory care patients with kidney disease. Book modules cover treatment guidelines from a variety of organizations and publications, as well as nonpharmacologic remedies.

The book contains three learning modules with a total available 14.5 continuing pharmacy education (CPE) credits. The content of Nephrologic and Geriatric Care was developed under the leadership of Faculty Panel Chair Emily R. Hajjar, Pharm.D., MS, BCPS, BCACP, BCGP.

Chapters in the first module focus on recognition and treatment of acute kidney injury and chronic kidney disease. The second module provides an overview of the approach to drug therapy in the setting of renal replacement therapy in chronic kidney disease and the management of polypharmacy in the older adult. The third module focuses on pharmacotherapy updates for neurocognitive disorder and falls in elderly patients.

Each ACSAP release includes two full-color online formats: (1) interactive PDFs you can save to your desktop or print; and (2) an e-media version you can view on an e-reader, tablet, and iOS or Android smart phone.

ACSAP chapters are fully referenced, with clickable hyperlinks to literature compilers such as PubMed.
