Heart Failure: Current and Emerging Management Strategies

 

Series Overview

From Theory to Bedside: Clinical Reasoning Series is one component of the professional development program developed by ACCP and approved by the Board of Pharmacy Specialties (BPS) for the recertification of the Board Certified Pharmacotherapy Specialist (BCPS). The 2015 program is also approved by BPS for the recertification of the Board Certified Ambulatory Care Pharmacist (BCACP). Clinical pharmacists who are board certified in both the pharmacotherapy and ambulatory care pharmacy specialties and who successfully complete the associated recertification test will receive credit toward recertification in both specialties.

This educational series explores cutting-edge, contemporary therapeutic topics, and demonstrates scientifically sound clinical reasoning and decision-making. It employs learning activities designed to advance the participants’ skills and abilities to critically evaluate the scientific evidence and to effectively incorporate into daily practice significant findings that lead to effective and safe patient care.

Program Content

“Heart Failure: Current and Emerging Management Strategies” focuses on contemporary strategies for the optimal management of patients with chronic and acute decompensated heart failure. Sessions detail and apply evidence that help inform individual patient care and advance pharmacist services.

Target Audience and Program Goals

The target audience for this program is clinical pharmacists, BCPSs, and BCACPs providing care to patients with heart failure.

At the conclusion of the program, the participant should be able to:

  1. Assess patients presenting with heart failure.
  2. Discuss current and emerging therapeutic options for the management of patients with chronic and acute decompensated heart failure.
  3. Discuss implications of chronic kidney disease on medication choice and management in patients with heart failure.
  4. Recommend evidence-informed, individualized plans for optimal management of patients with chronic or acute decompensated heart failure.
  5. Employ strategies to advance care processes, including transitions of care and pharmacist services.
Continuing Pharmacy Education Credit

ACPE

The American College of Clinical Pharmacy is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education.

This is an application-based activity; Activity Number: 0217-0000-15-111-H01-P.

To earn continuing pharmacy education (CPE) credit and to receive your continuing pharmacy education statement of credit for this activity, participants must successfully complete a Web-based posttest by October 14, 2018. Partial credit is not available for this activity.

BCPS/BCACP Specialty Recertification Credit

To be eligible for 6.0 CPE hours of recertification credit, BCPSs and BCACPs must successfully complete a Web-based posttest by November 30, 2015. Partial credit is not available for this activity.

ACCP, as the official provider approved by BPS, will record and report recertification credit earned for this activity directly to BPS.
