Clinical Reasoning Series in Pharmacotherapy – Part Two: Considerations for Anticoagulation in Unique Populations and Scenarios

Board Certified Pharmacotherapy Specialists are commonly involved with the decision of choosing the appropriate anticoagulation strategy in patients with unique indications such as left ventricular (LV) thrombus. In addition, pharmacists are also often impacted by drug shortages, supply chain issues, regulatory constraints, and medication safety considerations which influence the use DOACs and other anticoagulants. Part one of this program will explore the following topics: the current practice guideline recommendations in regards to prophylaxis and treatment of patients with LV thrombus and will assess the off label use of DOACs in this patient population. Part two of the program will provide an overview of the regulatory and safety considerations with anticoagulants. Both programs will focus on the role of anticoagulants in niche patient populations as well as the safety considerations and risk mitigation strategies with anticoagulant agents. Participants will have the opportunity to attend one or both educational activities. Each activity will offer 3.0 contact hours of continuing pharmacy education credit toward BCPS recertification credit upon completion of a required posttest by the submission deadline and achievement of a predetermined passing score. The posttest will be available on November 9, 2022. The deadline to submit posttests and earn recertification credit for the session will be November 9, 2023.
