Abstract

Introduction: Decreased kidney function can be classified into acute kidney injury (AKI), acute kidney disease (AKD), and chronic kidney disease (CKD). These patients are on multiple medications and need dose adjustment for renally-excreted medications. Although patients with decreased kidney function have a higher risk of having MTPs, evidence of MTPs specific to patients with decreased kidney function is not robust. Also, previous studies used various types of MTP classification systems and there is a lack of understanding of the rate of MTPs and the common MTPs among patients with decreased kidney function.

Research Question or Hypothesis: What is the extent of the literature in MTPs among adult patients who have decreased kidney function.

Study Design: Scoping Review

Methods: This scoping review will answer the following questions: Among adult patients with decreased kidney function, 1) What is the current evidence of MTPs? 2) What is the prevalence of MTPs? 3) Is there any difference in MTPs in terms of the type of kidney diseases and care settings? 4) Has there been published evidence describing the impact of pharmacists’ interventions against MTPs? 5) What are the gaps in the literature that exist regarding MTPs? PubMed, EMBASE, Cochrane Library, Web of science, International Pharmaceutical Abstracts, Google Scholar, and ClinicalTrials.gov will be searched for articles that included patients with decreased kidney function and reported MTPs. Any type of study designs and grey literature such as conference proceedings will be considered for inclusion. Articles that are non-English, non-human studies, pediatric participants only, and published before 1990 will be excluded. Data will be extracted by two independent reviewers, and any disagreements will be resolved by a third reviewer. All MTPs corrected will be re-categorized using PQA Medication Therapy Problem Categories Framework. Reporting will follow the Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews (PRISMA-ScR).
