Abstract

Introduction: Social determinants of health (SDoH) (e.g., education, clean water, health care access) are external factors shown to impact health outcomes. Inequities in SDoH influence the social vulnerability (SV) of a social group to the adverse impacts of natural hazards. Vulnerability has been linked to increased infectious morbidities, including Clostridioides difficile infections (CDI). The relationship between SDoH inequities and CDI has been described, however, there is a gap in the literature describing influence of SV on CDI outcomes.

Research Question or Hypothesis: Hospitalized patients with an initial CDI who have higher social vulnerability index (SVI) scores based on census tracts are more likely to be associated with worse clinical outcomes (i.e. mortality, intensive care unit length of stay (ICU LOS), presence of ileus, megacolon).

Study Design: The study is an IRB-approved, single-center, retrospective cohort study of adult patients admitted to Loma Linda University Medical Center (LLUMC) for an initial CDI episode from January 2020 to June 2021.

Methods: Adult patients admitted to Loma Linda University Medical Center (LLUMC) with an initial episode of CDI (ICD-10: A04.72) between January 2020 to June 2021 were included. Medical records were reviewed for demographics, comorbid conditions, insurance, total LOS, and need for ICU admission. Patients SVI data were collected by inputting their home addresses to the CDC/ATSDR SVI Interactive Map using data from 2020. Outcomes of interest include the SVI scores of 5 themes including overall SVI, socioeconomic status, household characteristics, racial & ethnic minority status, and housing & transportation as well as their corresponding level of vulnerability. Clinical outcomes of focus include all-cause mortality during admission, overall LOS, ICU LOS, rate of ICU admission, recurrent rate of CDI, CDI severity including presence of ileus and/or megacolon as identified on imaging, and receipt of infectious diseases (ID) or gastrointestinal (GI) consult.
