Mon-42 - A Comparison of Community Pharmacy Measured Blood Pressure to 24-hour Ambulatory Blood Pressure Monitoring: Preliminary Findings

Abstract

Introduction: It is unclear how community pharmacy measured blood pressure (CPBP) compares to ambulatory blood pressure monitoring (ABPM).

Research Question or Hypothesis: CPBP is equivalent to BP measured with ABPM during awake (daytime) periods.

Study Design: Prospective, single-arm, equivalency study

Methods: Eligible participants must be = 21 years of age with hypertension (HTN), taking =1 anti-HTN medication with no changes in the prior 2 weeks, and able to provide informed consent. We will exclude night shift workers, those with sleep apnea, a known arrhythmia, on dialysis, who are pregnant, or who have an arm circumference > 50cm. Participants will have 3 unattended seated blood pressure (BP) measurements at the pharmacy taken in both arms using the Omron HEM-907XL. Then, participants will be equipped with a SpaceLabs ABPM to record awake BP every 30 minutes. At the return visit, seated BP will be repeated and the mean of the six readings will serve as the CPBP. We plan to enroll 30 participants, which will yield 80% power at 5% significance. The 95% confidence interval (CI) for the mean difference between CPBP and mean awake ABPM will be estimated using a linear mixed model. BP will be the continuous outcome, modeled against a binary fixed effect for measurement device (CPBP vs. ABPM) and a subject-level random effect to account for the multiple BP readings arising from the same individuals. Equivalence will be considered if the 95% CI on the mean difference falls into the margins (-1.55, 1.55).

Results: Of 17 participants enrolled so far, 14 have complete data. The mean age is 65 years, 86% female, and 37% Black/African-American. The overall mean ± standard deviation for CPBP vs. ABPM is 124±18.3 mmHg vs. 124±13.4 mmHg for systolic BP and 72±9.6 mmHg vs. 74±6.2 mmHg for diastolic BP, respectively.

Conclusion: Preliminary results suggest CPBP may be equivalent to awake BP measured by ABPM.
