Older adults are at an increased risk for suboptimal medication management due to several factors including advanced age, frailty, and polypharmacy. Clinical pharmacists have the expertise to address and alleviate barriers to optimal medication use in older adults, and it is even further enhanced with an interprofessional healthcare team. This session aims to highlight best practices in comprehensive medication optimization for any clinical pharmacist when working with older adults through the development of novel clinical practice models in post-acute and long-term care services.
