This session is designed for clinical pharmacists, students, and residents who wish to gain knowledge related to health disparities and practical strategies to address health care gaps and improve health equity through medication optimization services for medically underserved and socially disadvantaged populations. The first presentation of this session will provide a foundation by defining important terms, providing evidence of health disparities, and describing clinical pharmacist opportunities and responsibilities to address health inequities. The second presentation will provide an example of successful implementation of comprehensive medication management (CMM) services within several federally qualified health centers. The third presentation will describe the implementation of telepharmacy services to overcome social determinants of health. The final presentation will describe a successful example of an economically sustainable model of CMM implementation within a Medicaid population.
