Providing Care for Patients with Rare Disease

This session will have 3 speakers who will present an overview for the management of rare diseases and the role of the pharmacist has in the care of this population.  The first presentation will be an over of rare disease state definitions and classification.  This session will serve as a primer for clinicians that are unfamiliar with rare diseases and how drugs are developed to manage these conditions.  The session will highlight resources that are available to both patients and clinicians. The second presentation will describe the role the pharmacist has in the provision of care for this population.  This program will describe strategies or techniques used by pharmacists specializing within specific rare diseases that can be used by all pharmacists. The third presentation will discuss the care transitions that are unique to this population including medication access, Risk Evaluation and Mitigation Strategy programs, specialty pharmacy, and inpatient/outpatient management.
