Pfizer Releases DHCP Letter and Updated Fact Sheet to Minimize Paxlovid Wrong Dose Medication Errors

Pfizer distributed an important prescribing and dispensing information letter to healthcare providers today drawing attention to reports of wrong-dose medication errors that have occurred with Paxlovid. The errors occurred during prescribing, dispensing, and administration. Pfizer reports that incorrect patient self-administration accounted for many of the errors and generally reflected patients incorrectly taking the wrong combination of nirmatrelvir tablets and ritonavir tablets from the blister packaging.
 
The letter reminds clinicians about the need for individualized dosage adjustments and the availability of two different nirmatrelvir dosage strengths prepackaged in blister packs. Additionally, Pfizer revised the Paxlovid Patients, Parents and Caregivers Fact Sheet to address wrong dose medication errors that occur during patient self-administration.
