Abstract

Introduction: Healthcare costs for diabetic patients are estimated to be more than double those of non-diabetic patients. Glucagon-like peptide-1 receptor agonists (GLP-1 RA) provide glucose control, weight loss, and cardiovascular protection; however, therapy comes with significant costs. Cost effectiveness of therapy remains unclear, so we aimed to evaluate the cardiovascular outcomes and cost effectiveness within our patient population.

Research Question or Hypothesis: Do cardiovascular benefits associated with GLP-1 RA’s outweigh additional drug costs in patients with type 2 diabetes mellitus (T2DM)?

Study Design: This retrospective cohort study was comprised of patients managed by pharmacists for the treatment and management of T2DM between January 1, 2020, and June 30, 2023, at SSM Health Monroe Clinic or Dean Medical Groups sites.

Methods: Patients were identified using the system’s electronic health record data extraction tool. Adults with T2DM and at least one antidiabetic medication on their medication list were included. Two study cohorts were identified; those patients receiving GLP-1 RA therapy and those who were not. Financial outcomes were evaluated using a subgroup of patients with a selected insurance plan. Primary outcomes included relative risk of composite CVD and relative risk of all-cause mortality. The subgroup’s primary outcome was per member per month (PMPM) costs.

Results: Of the 2489 patients in clinical analysis, 1033 received GLP-1 RA therapy and 1456 did not. The risk of composite cardiovascular disease (MI, stroke, HF) was significantly decreased among GLP-1 RA patients (RR 0.6069; 95% CI 0.5594 to 0.6586). GLP-1 RA therapy significantly reduced the risk of all-cause mortality (RR 0.5384; 95% 0.3852 to 0.7527). 1670 patients carried the selected insurance plan and were included in subgroup analysis. Internal cost analysis with SSM payors has been completed but cannot be shown due to IRB restrictions.

Conclusion: The cardioprotective effects of GLP-1 RA therapies likely outweigh the risks and costs associated with therapy.
