Abstract
Introduction: Management of anticoagulation therapy to optimize patient outcomes and minimize adverse events is a pivotal concern in patient care, necessitating meticulous coordination among healthcare professionals.
Research Question or Hypothesis: This study evaluates the effectiveness of Anticoagulation Stewardship Programs (ASPs) in Saudi Arabia, with a focus on pharmacist-led interventions aimed at enhancing the appropriateness of anticoagulation regimens and the rate at which physicians accept such recommendations.
Study Design: A multicenter retrospective analysis was conducted from January to December 2023 across Ministry of Health hospitals that implemented ASPs.
Methods: Participants consisted of adult patients receiving oral or intravenous anticoagulation therapy, excluding specific demographics. Comprehensive data covering 9338 pharmacist interventions were collected from Electronic Health Records. Descriptive statistics and binary logistic regression analyses were used to categorize interventions and evaluate their acceptance.
Results: The analysis demonstrated that among the patients who needed intervention (38.8%), physicians accepted an impressive 96.1% of the recommendations made by pharmacists.
Conclusion: This study highlights the critical role of pharmacists within ASPs and potential areas for improvement in interdisciplinary collaboration. Although this work did not assess the direct impact of pharmacist interventions on patient outcomes, the findings call for the expansion of ASPs to foster a more integrated, patient-centered healthcare approach in Saudi Arabia. This research lays important groundwork for future studies to explore the direct effects of pharmacist interventions on patient outcomes and ultimately strengthen the pharmacist's role in anticoagulation therapy management.
