Mon-41 - Real-world Changes in Weight Following the Discontinuation of GLP-1 and Dual GLP-1/GIP Receptor agonists

Abstract

Introduction:

Glucagon-like peptide 1 (GLP-1) and dual Glucagon-like peptide 1/Gastric inhibitory polypeptide (GLP-1/GIP) receptor agonists have become preferred agents in the management of diabetes and obesity. However, they can have adverse effects, carry potential financial burdens to patients, and continue to be on periodic shortage, which can result in patient discontinuation. Much of the data regarding changes in weight following discontinuation of these agents is in the context of clinical trials, which may not accurately reflect real world conditions.

Research Question or Hypothesis:

How will weight change in patients treated with a GLP-1 or dual GLP-1/GIP RA following discontinuation of the agent?

Study Design:

Descriptive retrospective cohort study

Methods:

This study was based on pharmacy dispensing and electronic health record (EHR) data from 01/01/2019 to 04/01/2024. Patients who had received at least 84 days supply of GLP-1 or dual GLP-1/GIP RA were identified. Patients were included if they had a gap of at least 180 in days' supply covered, were =19 years old on index date, had a diagnosis of diabetes or obesity, and review of the EHR confirmed discontinuation. The primary outcome is weight change from date of discontinuation to 180 days post discontinuation. Additionally, demographics data, diabetes and obesity related comorbidities, changes in A1C, and reason for discontinuation were recorded.

Results:

Data was collected on 275 participants. 65.1% were female. 11.6% were Hispanic or Latino. 17.1% were Black/African American, 71.6 were White/Caucasian, 2.2% were another race. The average age was 51(SD 14.03) years old. The most common reasons for discontinuation were adverse effects followed by insurance formulary concerns. Research is in progress for the outcomes surrounding weight.

Conclusion:

When compared to the racial/ethnic makeup of the general Nebraska population, the initial sample is a good representation. No conclusions about weight have been drawn yet.
