Abstract

Introduction: Hepatorenal syndrome-acute kidney injury (HRS-AKI) is a severe complication of end-stage liver disease resulting in rapidly progressive renal failure. Terlipressin is a synthetic vasopressin analogue recommended as first line treatment for HRS-AKI. The CONFIRM trial demonstrated that combination albumin and terlipressin were effective in reversal of HRS-AKI. Data from end-stage liver disease patients in the clinical setting has not been described. The purpose of the study is to assess the efficacy and safety of terlipressin in reversing HRS-AKI in institutions across the United States. This data will aid in identifying optimal place in practice for terlipressin.

Research Question or Hypothesis: Terlipressin effectively increases rates of HRS-AKI reversal in end-stage liver disease patients in the clinical setting.

Study Design: Multicenter, retrospective, observational study

Methods: This multicenter, retrospective, observational study aims to assess the efficacy and safety of terlipressin in adult patients with HRS-AKI. All sites were recruited through the Vizient Pharmacy Network and Research Committee and American College of Clinical Pharmacy Practice and Research Networks. Patients will be included if they are 18 years of age or older with a diagnosis of cirrhosis with HRS-AKI and received terlipressin during a hospital admission between September 14th, 2022 and March 31st, 2024. The primary outcome is rate of HRS-AKI reversal. Secondary outcomes include safety of terlipressin, recurrence of HRS-AKI, rates of renal replacement therapy, transplantation, rehospitalization, and death, as well as description of terlipressin administration.
