Available for 1.50 hours of CPE creditActivity Number: 0217-0000-25-137-L01-PActivity Type: A Knowledge-Based ActivityAs the population ages, pharmacists increasingly encounter older adults with complex medical comorbidities requiring antithrombotic therapy for conditions such as atrial fibrillation, venous thromboembolism, and atherosclerotic cardiovascular disease. While these therapies reduce the risk of thrombotic events, they also carry significant risks, including bleeding, drug interactions, and reduced quality of life. The 2023 American Geriatrics Society (AGS) Beers Criteria® includes multiple updates to its recommendations regarding potentially inappropriate medications related to antithrombotic agents. The goal of this program is to equip pharmacists with evidence-based strategies to optimize antithrombotic therapy in the older adult population. Participants will explore the unique physiological and clinical considerations in older adults influencing safe and effective antithrombotic prescribing. Special attention will be given to deprescribing practices, including identifying patients who may no longer benefit from therapy.
Learning Objectives1. Review updated recommendations regarding antithrombotic medications per the 2023 AGS Beers Criteria®.
2. Identify antithrombotic medications that are potentially inappropriate for older adults.
3. Identify risk factors for bleeding and thrombosis in an older adult patient.
4. Outline safe and effective antithrombotic medication use among older adults.
5. Recognize situations where an older adult patient may no longer benefit from antithrombotic therapy.
6. Describe strategies for appropriate deprescribing of antithrombotic agents.
