Pharmacists play an integral role in agent selection, dosing, monitoring and optimizing anticoagulant therapy. This session examines new guidelines and literature that will inform future practice management of anticoagulants in selected populations.
This session will discuss the ISTH guidance document’s patient-centered approach to optimize outcomes while minimizing risks, emphasizing the importance of individualized treatment plans while considering the severity of liver disease, bleeding risk, and patient preferences. Additionally, this session will review updated guidance to equip healthcare professionals with the knowledge and tools needed to effectively manage bleeding complications in patients on DOAC therapy, ensuring patient safety and optimal outcomes. The final presentation in this session will address late-breaking articles that provide additional evidence to the current practice of anticoagulation selection, dosing, monitoring, and adjustment in children.
Learning Objectives1. Review the recent guidelines for anticoagulation in stroke prevention for atrial fibrillation, treatment of venous thromboembolism and portal vein thrombosis in patients with cirrhosis.
2. Summarize the latest guidelines for the effective and safe reversal of direct oral anticoagulants in clinical practice.
3. Discuss the updated guideline for treatment of venous thromboembolism in pediatric patients.
4. Recognize clinical applications for the latest publications in anticoagulation management into clinical practice.
