Attendees will gain immediately applicable strategies to optimize therapy, reduce clinical inertia, minimize hypoglycemia risk, and align diabetes management with patient-specific goals of care. Through real-world case scenarios, a repeatable interpretation framework, and practical communication tools, participants will leave with increased confidence and clarity in applying CGM data to support consistent, patient-centered clinical decision-making in ambulatory practice.
This activity is approved for Board Certified Ambulatory Care Pharmacist (BCACP) and Board Certified Geriatric Pharmacist (BCGP) recertification credit.
The BCACP and BCGP Clinical Sessions are part of the professional development program for the recertification of board-certified Ambulatory Care or Geriatric pharmacists by the Board of Pharmacy Specialties and jointly provided by ACCP and the American Society of Health-System Pharmacists (ASHP). In order to earn the BCACP, BCGP recertification credit, participants must attend the session, claim the continuing pharmacy education credit, and pass the associated posttest. Access to the Clinical Sessions posttests will be available on December 16, 2026, at www.accp.com/myaccount to anyone who has purchased access to the BCACP/BCGP posttests. For participants who have not purchased access to the posttests, access can be purchased until 12:00 p.m. on Tuesday, October 20, at the ACCP Registration Desk. The deadline to submit posttests for these sessions will be December 14, 2027.
Learning Objectives1. Design a structured approach to reviewing continuous glucose monitoring (CGM) reports to efficiently identify actionable glucose patterns in clinical diabetes management.
2. Evaluate CGM metrics to prioritize medication therapy decisions that improve glycemic outcomes while minimizing hypoglycemia risk in adults and older adult patients.
3. Practice integrating CGM data into pharmacist-driven clinical decision-making to individualize diabetes therapy intensification or de-escalation.
4. Apply CGM data to engage patients in shared decision-making and goal setting that supports safe, patient-centered diabetes management and reduces clinical inertia.
