This session aims to provide a practical, evidence-informed overview of antithrombotic medications in pediatric patients, a population where thrombosis risk is rising, but high-quality pediatric data remain limited. The session focuses on indications, agent selection (anticoagulant and antiplatelet agents), dosing considerations, monitoring, and safety in common pediatric scenarios such as congenital heart disease, central venous catheters, postprocedural care, and transitions of care. Participants will gain confidence in translating emerging evidence and recently updated guideline recommendations into real-world clinical decision-making.

This activity is approved for Board Certified Pediatric Pharmacy Specialist (BCPPS) recertification credit.

The BCPPS Clinical Sessions are part of the professional development program for the recertification of board-certified Pediatric pharmacists by the Board of Pharmacy Specialties and jointly provided by ACCP and the American Society of Health-System Pharmacists (ASHP). To earn BCPPS recertification credit, participants must attend the session, claim continuing pharmacy education credit, and pass the associated posttest(s). Access to the Clinical Session posttests will be available on December 16, 2026, at www.accp.com/myaccount to anyone who has purchased access to the BCPPS posttests. For participants who have not purchased access to the posttests, access can be purchased until 12:00 p.m. on Tuesday, October 20th, at the ACCP Registration Desk. The deadline to submit posttests for these sessions will be December 14, 2027.  

Learning Objectives

1. Apply evidence-based antithrombotic medication principles to select, dose, and monitor therapy for pediatric patients in common clinical scenarios.

2. Identify transitions-of-care challenges related to dosage formulation availability and medication administration to ensure safe and effective therapy in pediatric patients.

3. Interpret key updates from the recent thrombosis treatment and thromboprophylaxis guidelines and apply them to clinical practice.

4. Design individualized antithrombotic therapy management plans for pediatric patients with congenital heart disease.

5. Explain the principles and goals of antithrombotic stewardship in pediatric practice and the role of Hemostasis and Anticoagulation (HAT) teams.
