Available for 1.50 hours of CPE creditActivity Number: 0217-0000-26-156-L01-PActivity Type: A Knowledge-Based ActivityThe older adult population in the United States continues to rise with the number of Americans aged 65 years and older estimated to be 95 million by 2060. The need for excellent training in geriatric care is becoming increasingly urgent. This continuing education program will explore the impact of Center for Medicare & Medicaid Services (CMS) Age Friendly Hospital Measure and the Hospital Inpatient Quality Reporting (IQR) Program on clinical pharmacy practice in health-system settings. This program will also give an overview of the 5Ms framework which promotes the well-being of older adults. Learners will apply “Multicomplexity,” “What Matters,” “Medication,” “Mentation,” and “Mobility” in evaluating geriatric patients to identify potential drug related problems and to improve therapy-related clinical outcomes (avoiding use of potentially inappropriate drugs, reducing falls and delirium, addressing psychosocial/health disparity issues to promote medication adherence, etc.). Delivering age friendly care using the 5M framework aims to reduce hospital re-admission, decrease length of stay and improve health outcomes in older adults. Each speaker will use assessment questions throughout the presentation with an audience response system. Each speaker may provide insight on applying 5Ms framework in a patient case study that will prompt learners to apply knowledge gained. The questions and/or summary patient cases will also serve as assessment tools to evaluate the achievement of learning objectives.
Learning Objectives1. Summarize key areas of the 5Ms framework which promotes the well-being of older adults.
2. Describe the new Center for Medicare & Medicaid Services (CMS) Age Friendly Hospital Measure and the Hospital Inpatient Quality Reporting (IQR) Program.
3. Identify potentially inappropriate medications that may increase risk for delirium and falls in older patients.
4. Explain the process of applying “Mentation” and “Mobility” in therapy optimization for older patients in peri-operative settings.
5. Identify psychosocial issues that increase risk for hospital readmission in older adults.
6. Explain the process of applying “Medication” and “What Matters” in therapy optimization for older patients at hospital discharge
