This session uses case-based discussion to examine gray areas encountered when applying race-agnostic kidney function estimates in practice. Emphasis will include implications of moving away from Cockcroft-Gault, patient-specific considerations, and challenges in implementation across care settings. Faculty will connect emerging consensus recommendations with practical decision-making strategies that prioritize accuracy, patient safety, and equity.
Learning Objectives1.Compare strengths and limitations of commonly used kidney function estimation methods in clinical pharmacy
practice.
2. Analyze approaches to medication decisions when kidney function estimates are discordant or uncertain using patient cases in ambulatory care and inpatient adult medicine.
3. Apply evidence-informed strategies to optimize medication dosing while promoting safety and equity.
4. Identify implementation considerations for integrating race-free eGFR into clinical and institutional workflows.
