In August 2026, the Primary Care Collaborative (PCC) and the American College of Clinical Pharmacy (ACCP) collaborated to develop a new resource guide supporting the integration of Comprehensive Medication Management (CMM) into advanced primary care models, including the Patient-Centered Medical Home (PCMH). The guide highlights the critical role of clinical pharmacists in optimizing medication use, improving patient outcomes, and strengthening team-based care.
CMM is a patient-centered, team-based approach that ensures medications are used appropriately, effectively, and safely. With medications involved in approximately 80% of all treatments and medication-related problems costing the U.S. healthcare system more than $200 billion annually, the resource underscores the importance of achieving medication optimization through comprehensive medication management as a core component of advanced primary care.
The fact sheet provides several resources for implementing and advancing CMM, including foundational resources that make the case for CMM and standardize its core components. These resources have come from ACCP-supported initiatives like the CMM in Primary Care study and the Get the Medications Right Institute (GTMRx), in addition to ACCP’s continued collaboration with the PCC as a longtime executive member.
Broader adoption of CMM can deliver value across the healthcare ecosystem by improving outcomes and experiences for patients, reducing clinician burden, lowering costs for payers and employers, and decreasing total cost of care for health systems. PCC and ACCP emphasize that sustainable payment and policy support will be essential to expanding access to CMM and realizing the full potential of advanced primary care.
