Description:

The past decade of research in patients with heart failure has seen many therapeutic advances. A majority of this research has focused on optimization of pharmacotherapy in patients stable for hospital discharge or those in the community setting. Management and initiation of guideline-directed medical therapy (GDMT) for critically ill patients with heart failure in the intensive care unit is less well characterized and much of the recommendations are extrapolated from the outpatient setting or expert recommendations. There is a growing body of literature on how to best optimize pharmacotherapies while patients are critically ill. However, a uniform consensus on optimization in the acute setting is lacking. Less evidence is available to best guide the management of acute right ventricle failure. Decongestion strategies continue to evolve and new information continues to guide decongestive strategies beyond loop diuretics.

This program looks to characterize evidence-based pharmacotherapy in critically ill patients with heart failure with reduced ejection fraction (HFrEF), acute right ventricular failure, and/or diuretic resistance.

 

Learning Objectives:

  1. Interpret clinical criteria to differentiate between left and right heart failure
  2. Assess current evidence for implementation of guideline-directed medical therapy (GDMT) in the ICU
  3. Develop a pharmacotherapy plan to overcome acute right heart failure
  4. Differentiate the use of diuretic strategies to overcome diuretic resistance
