With expanding use of antiplatelet therapies in neurocritical care, pharmacists are increasingly challenged to navigate complex transitions between parenteral and oral agents in this high-risk population. Despite growing reliance on P2Y12 and aspirin platelet function assays, their optimal use, clinical significance, and interpretation remain debated in the literature. Decisions regarding when to order assays, how to interpret results, and how to adjust therapy are still evolving. This program will emphasize interpreting platelet function assays, managing therapy transitions, and developing individualized antiplatelet strategies in complex neurovascular patients.
Learning Objectives:
- Describe commonly utilized testing for the monitoring of antiplatelet therapies.
- Summarize the transition from parenteral and oral antiplatelet therapy, including the significance of loading doses and the consideration of dual or triple antiplatelet therapy.
- Outline a treatment plan for patients in the neurocritical care setting who may benefit from therapeutic drug monitoring of antiplatelet therapy.
