Beta-lactam antibiotics remain cornerstone therapies for the management of severe infections in critically ill patients; however, standard dosing approaches frequently fail to achieve optimal pharmacokinetic and pharmacodynamic (PK/PD) targets in this population. Physiologic derangements associated with critical illness, including altered volume of distribution, dynamic renal function, hypoalbuminemia, and the use of extracorporeal support, can result in highly variable beta-lactam exposure, increasing the risk of both treatment failure and toxicity. Therapeutic drug monitoring (TDM) has emerged as a potential strategy to individualize beta-lactam dosing, yet questions remain regarding its clinical evidence, optimal application, and feasibility in routine practice. This 60-minute collaborative educational session between the Critical Care and Infectious Diseases PRNs will explore the evolving role of beta-lactam TDM in critically ill patient populations.

pharmacodynamics of beta-lactam antibiotics.
- Summarize the current evidence evaluating beta-lactam therapeutic drug monitoring in critically ill patients,
including reported efficacy and safety outcomes.
- Discuss key implementation and practical considerations for beta-lactam therapeutic drug monitoring.
- Explain key principles and considerations used to inform beta-lactam dosing and monitoring strategies in
critically ill patient populations.
