On May 20, the Primary Care Collaborative (PCC) hosted a webinar to show how a team-based, whole-person approach to care could leverage the expertise of primary care, behavioral health, and clinical pharmacy to better support patients with chronic conditions.
The webinar explained the importance of coordinated care, explored strategies for improving and fostering stronger collaborations, and discussed potential solutions policymakers could put in place to foster team-based approaches to care management. BatemanThomas Bateman, Pharm.D., BCACP, is a clinical assistant professor with Rutgers, Ernest Mario School of Pharmacy. He currently serves as the lead clinical pharmacist of Henry J. Austin Health Center, a federally qualified health center in Trenton, New Jersey. He also serves as the residency program director for the PGY2 ambulatory care residency program. Bateman’s passion is working with urban communities that are medically underserved, and he has published multiple papers, mainly about chronic disease management in the outpatient setting.
ACCP has been an executive member of the PCC for more than 10 years and will continue to collaborate with this organization to advance comprehensive medication management in primary care. ACCP would like to acknowledge and thank Dr. Bateman for his participation in this webinar.
