Robust evidence demonstrates that collaborative practice between clinical pharmacists and physicians positively influences all dimensions of the Quintuple Aim—enhancing patient experience, improving population health, reducing costs, supporting care team well-being, and advancing health equity. Team-based care models foster integrated, person-centered, and coordinated patient care delivery. Collaborative Practice Agreements (CPAs) offer a structured framework that delineates and supports the complementary roles of clinical pharmacists and physicians within these models. To facilitate the adoption of comprehensive clinical pharmacy services in team-based settings, the American College of Clinical Pharmacy (ACCP) has developed a CPA template designed to streamline implementation and promote best practices.
