Community Care Physicians and Albany College of Pharmacy and Health Sciences
Community Care Physicians, P.C., also known as CCP, is the largest, independent multispecialty medical group in the Capital Region of New York State. CCP addresses the individual needs of patients seeking primary care, specialty care, urgent care, and wellness services. The organization includes more than 420 practitioners across 80 locations and 30 specialties. The pharmacy department is made up of five clinical pharmacists and a PGY2 resident. The clinical pharmacists work as members of the health care team to assist in medication and disease state management within our primary care offices. The pharmacists focus on both direct patient and population health activities to improve care for our patients. After an orientation and training period, PGY2 residents become embedded in their own practice, where they can work directly with other health care providers and build their own patient panel as independent practitioners.
Albany College of Pharmacy and Health Sciences is a private, independent institution with a long tradition of academic and research excellence. The college uses a variety of technology to allow both in-person and virtual teaching. PGY2 residents have the opportunity to teach in both the classroom and the experiential setting. They learn about the roles and responsibilities of being a faculty member within a college of pharmacy and fine-tune their teaching philosophy. There are also many opportunities to present CE-approved presentations and participate in Ambulatory Care Grand Rounds with other PGY2 residents for board certification preparation.
The PGY2 resident’s longitudinal learning experiences are family medicine, academia/teaching practice management and population health, and research/scholarship. Other required rotations are chronic disease state management, comprehensive medication management, and pharmacoeconomics led by experienced clinical pharmacists as preceptors. In addition, elective rotations can be tailored to the resident’s needs; those offered include advanced academia, pediatrics, and substance use disorder.
Life outside residency is definitely filled with adventures around this capital city. There are various entertainment venues, with places to hike in the summer or ski in the winter. The region and its surrounding cities also have diverse restaurants and many breweries to explore on weekend trips. Not only is Albany a weekend trip away from New York City, but multiple states can also be visited in 2–3 hours.
