Managing Cardiovascular Therapies in the Critically Ill

 

Series Overview

Clinical Reasoning Series in Critical Care Pharmacy has been approved by the Board of Pharmacy Specialties for recertification of Board Certified Critical Care Pharmacy Specialists (BCCCPs).

BCCCPs are involved in managing cardiovascular related medication therapies. They are often responsible for assessing and developing drug therapy regimens for critically ill patients with acute cardiovascular related complications or supportive devices. Acute heart failure, thrombosis, bleeding and increasing application of devices have become common challenges in the intensive care unit. Critically ill patients with notable cardiovascular conditions are at high risk for organ related complications, adverse medication events and related morbidity and mortality that in many cases can be avoided. As our understanding of more optimal approaches to assessment tools and how to approach disease state management evolves, BCCCPs need to engage as prospective as possible using evidence-based strategies that frequently have to be individualized to the complexity and frequently understudied critically ill patient. The complexity of managing these conditions and common occurrences of special populations, this program will address current evidence-based strategies and summarize their place in contemporary patient care. The program will consist of several educational activities and include practice skills, current evidence and management approaches.

Program Content

Topics to be covered include acute heart failure and related support strategies, atrial fibrillation, thrombosis management, coagulopathies, hemorrhagic emergencies and anticoagulation reversal. Each activity will discuss treatment strategies and application of skills to knowledge in developing management plans.

Target Audience and Learning Objectives

Clinical Pharmacists, and BCCCPs providing care to patients with acute heart failure and related support strategies, atrial fibrillation, thrombosis management, coagulopathies, hemorrhagic emergencies and anticoagulation reversal.

At the conclusion of Part 1 of the program, the participant should be able to:

  • Describe skills to consider in initiating individualized pharmacotherapy
  • Describe the risk assessment and initiating a antithrombotic prophylaxis or treatment management plan
  • Describe when and how to approach the pharmacologic management of Atrial Fibrillation
  • Explain the process of evaluating and initiating Thrombolytic therapy (non-Stroke setting)
  • Describe the impact of unique patient characteristics that may lead to modified management plans in the setting of going into and out of acute Heart Failure
  • Identify pharmacologic agents used in the acute management and impact of acute failure on other therapies
  • Describe considerations for individualizing therapy in special populations

At the conclusion of Part 2 of the program, the participant should be able to:

  • Describe skills that can be utilized to measure and adapt therapy to meet management goals
  • Describe selected coagulopathic situations including liver failure and HIT
  • Identify various agents used to reverse the effects of anticoagulants and antiplatelet agents
  • Explain how anticoagulants and Antiplatelet agents are managed around invasive procedures
  • Describe considerations for managing thrombosis and bleeding in patients with a LVAD/Impella or cardiac valve
  • Explain the ECLS circuit and pharmacologic management considerations

ACPE Continuing Pharmacy Education Credit

BCCCP Credit
Recertification Credit

 

To be eligible to earn recertification credit, BCCCPs must purchase the recertification version of this course, successfully complete the course, and submit and pass the web-based posttest associated with each module by July 10, 2020. Partial credit is not available for this activity. Posttests may not be submitted more than one time.

The passing point to earn BCCCP recertification credit is based on an expert analysis of the assessment items in each posttest module. Any posttest submitted before the BCCCP posttest deadline that meets this passing point will earn BCCCP recertification credits. These credits will be assigned as of the date of test submission and reported within 48 hours to BPS. For statements of recertification credit, visit www.bpsweb.org.

All participants must have a Board of Pharmacy Specialties (BPS) board certification credential number, BPS ID Number, and a BPS expiration date on file with ACCP for reporting purposes.
